How to Make an Appointment

The Practice appointment system provides appointments which can be booked on the day and in advance. Our CARE NAVIGATORS (receptionists) have been trained in care navigation and are here to help to you access information and advice. Our doctors have asked them to ask you some questions to ensure you see the most appropriate person to help you. This may be a doctor, a nurse, a HCSW (Healthcare Support Worker), a physiotherapist , a GPSO (GP Support Worker). You may be offered a telephone consultation, a video consultation or a face to face consultation.

NHS WALES APP

We are pleased to announce that you can now access health services from Practice 3 on the new NHS Wales App.

The App is a simple and secure way to:

· book routine appointments

· order repeat prescriptions

· view parts of your medical record

To access the App you must:

· have a fully verified NHS login, or a valid photo ID to prove who you are

· be aged 16 or over

Appointments with a doctor, our Practice Nurse, Nicola or our HCSW (Healthcare Support Worker), Sarah can be booked online or by calling the Practice and speaking to our are care navigators.  The care navigator will ask you the nature of your illness to ensure you are directed to the most suitable clinician. Our care navigators have all participated in signposting training and will handle your call confidentially and with sensitivity. Please let the care navigator know if you require additional assistance and will make arrangements to book you in at a suitable time. We have access to a translation service on site for our patients we will make adjustments for people’s sensory, communication, language and other needs, including making online services compatible with screen-readers.

Contacting us Digitally

Please use eConsult which is our Consult Your Doctor Online service on our homepage. Click Here for eConsult which is a secure way access self- help information or contact the Practice for the following

  • Advice from a clinician for medical conditions such as a rash, sore throat etc. Please note we respond by the end of the following day or sooner. If your condition becomes worse or you are concerned while waiting to hear from us, please phone the Practice.
  • Requests for sick notes
  • Requests for information of medical or insurance reports
  • Questions about a referral
  • Other administrative questions
  • We may ask you to submit a review for asthma, diabetes, contraception etc. using eConsult

Please cancel your appointment if you no longer require it, this allows another patient to be seen

Urgent Appointments

If you require an urgent appointment please call to speak to the receptionist /care navigator so you can be prioritised appropriately.

Home Visits

Whilst we encourage our patients to come to the surgery, where we have the proper equipment and facilities available, we do appreciate this is not always possible. In this respect, if you do need a home visit, you can help us by calling 01685 351493 before 10.00am.

You may only request a home visit if you are housebound or are too ill to visit the practice. Our GP will only visit you at home if they think that your medical condition requires it and will often phone you first to decide how urgently a visit is needed.

Video Consultations

We may offer you a video consultation if it is appropriate for your medical condition. Please read the patient information sheet for instructions.
