Introduction
Welsh Government have asked all GP practices in Wales to support patients in 3 areas of unhealthy behaviour as part of their strategy for a ‘Healthier Wales’. The key focus is is on health prevention and making every contact count. If you have contacted us in recent months, you will have been asked the following questions and offered the support as below, particularly if you are a new patient or you have a chronic health problem such as diabetes, asthma, COPD, hypertension and cardiovascular disease.

Do you smoke? If you do the amount you smoke or vape | You may be given advice in the Practice or Signposted to ‘Help me Quit Wales’ Our online consultation service eConsult also automatically sends a link to ‘Help me Quit Wales’ |
Do you drink alcohol? If you do the amount you drink weekly | You may be given lifestyle advice and a leaflet which is below in the alcohol section or Referred to CDAT (Community Drug and Alcohol Team) |
To share your current height & weight with us | You may be given lifestyle advice and tips on diet, or offered to start a weight management programme, you may be referred for a local service |
We are sending new patients and patients with a chronic disease a text message with a link to this page. If you have any questions or would like lifestyle advice in the Practice, please make an appointment with Sarah, our Healthcare Support Worker. These appointments are available in the afternoon on Mondays – Thursdays. You can use the NHS Wales APP to book your appointment . The NHS Wales APP also has direct link to Healthy Weight.
