If you wish to register with Practice 3, please telephone the receptionist
on 01685 351493 or visit reception for details. You will be asked to complete a registration form and a new patient health questionnaire and given further instructions by the receptionist, to complete the registration process.
It will speed up the process if you print the registration form below and
all family members complete and bring to the Practice. We suggest you register for online services at the same time.
Please let us know any changes to your personal details . You can email us changes to your address or contact numbers.
If you change your name please bring evidence such as a copy of your marriage certificate or deed poll
